Bereavement and Volunteer Coordinator
The Bereavement and Volunteer Coordinator is responsible for the coordination and provision of grief support and education to hospice patients, families and community members. This includes one-on-one support, group support meeting facilitation, community education and coordination of memorial events. This position is also responsible for recruiting, training assigning and evaluating volunteer services used for patient care and administrative needs.
Bachelor's or higher degree in social work, psychology, thanatology, sociology, counseling or related field preferred. Certification in grief and bereavement support preferred or willingness to obtain within one year of employment.
On-Call Registered Nurse
The on-call Registered Nurse is responsible for meeting the after-hours/on-call needs of patients and families. The on-call RN will ensure that care and services are delivered appropriately and timely during after-hour and holiday coverage. The RN will receive patient related calls, determine the needs of the caller and then respond in appropriately, either by visit or phone. Frequency and scheduling of shifts is flexible and based on candidate availability. WY RN license required.
Case Manager Registered Nurse
Part-Time Position with Benefits. Potential to become a full-time position. The Case Manager Registered Nurse plans, coordinates and provides care to meet the physical, emotional and spiritual needs of hospice patients in Sweetwater County. Schedule includes on-call rotation. WY RN license required.
