Full-Time Position. The Case Manager Registered Nurse plans, coordinates and provides care to meet the physical, emotional and spiritual needs of hospice patients in Sweetwater County. Schedule includes on-call rotation. WY RN license required. New Graduate applications accepted.
The on-call Registered Nurse is responsible for meeting the after-hours/on-call needs of patients and families. The on-call RN will ensure that care and services are delivered appropriately and timely during after-hour and holiday coverage. The RN will receive patient related calls, determine the needs of the caller and then respond in appropriately, either by visit or phone. Frequency and scheduling of shifts is flexible and based on candidate availability. WY RN license required. 1-2+ years nursing experience preferred.
Hospice of Sweetwater County is currently seeking a Certified Nursing Assistant (CNA) to join our team. The hospice CNA provides personal care services to hospice patients under the direction and supervision of a Registered Nurse Case Manager. This position is part-time (20-30 hours per week) with potential to become full-time. WY CNA license required. Experience preferred.
